BACKGROUND/AIMS: Although many regimens, including quadruple, sequential, and concomitant treatment, are used and recommended as first-line or rescue therapies for Helicobacter pylori infection, eradication rates are still below 90% in intention-to-treat analyses. Treatment protocols with substantially high eradication rates and low antibiotic resistance are needed. In this study, we investigated the efficacy of high-dose dual therapy as first-line treatment in a Turkish population. MATERIALS AND METHODS: All patients underwent upper gastrointestinal endoscopy for the initial H. pylori status because of dyspeptic symptoms. All patients received a 14-day, high-dose dual therapy comprising rabeprazole (20 mg t.i.d.) and amoxicillin (1 g t.i.d.) for H. pylori eradication. H. pylori stool antigen tests of eradication were administered to all participants at least 4 weeks after the completion of the treatment. RESULTS: The high-dose dual therapy demonstrated a 91.3% rate of successful eradication of H. pylori infection. Per-protocol success was 94.4% among female patients (n=51) and 89.6% among male patients (n=86); in terms of gender, the differences were not significant (p=0.310). No side effects were observed during the study in any patient. Six other patients did not take adequate doses of the treatment protocol. CONCLUSION: High-dose dual therapy with rabeprazole and amoxicillin was highly effective and well tolerated as a first-line therapy for H. pylori eradication. Sofosbuvir plus ledipasvir (SOF-LDV) combination therapy is a promising therapy for post-transplant hepatitis C virus (HCV) reinfection. It is known that gastric pH elevation induces lower absorption of ledipasvir; therefore, the use of proton pump inhibitors (PPIs) should be considered regarding dose reduction after SOF-LDV therapy induction. Here, we report two patients who developed duodenal ulcers due to the discontinuation of PPIs after the induction of SOF-LDV therapy for post-transplant HCV reinfection. The first patient was a 71-yr-old man who had undergone living donor liver transplantation due to HCV-related liver cirrhosis. Lansoprazole, 30 mg daily, was discontinued upon SOF-LDV therapy induction. Seven days after SOF-LDV therapy induction, gastrointestinal endoscopy revealed the presence of a duodenal ulcer. The second patient was a 54-yr-old man who had undergone living donor liver transplantation due to HCV-related end-stage liver disease. Similar to the first patient, rabeprazole sodium was discontinued upon the induction of SOF-LDV therapy. Eighteen days after SOF-LDV therapy induction, gastrointestinal endoscopy revealed the presence of a duodenal ulcer. Regimen efficacy was evaluated by per-protocol (PP) and intention-to-treat (ITT) analyses. One hundred eighteen patients were included in the study. The eradication rate of RADB was 93.8% (105/112; 95% CI 89.2%-98.3%) in PP anal. and 89.8% (106/118; 95% CI 84.3%-95.4%) in ITT anal. The patient compliance rate was 97.5% (115/118). The eradication rate of Helicobacter pylori (H. pylori) has been decreasing every year, mainly due to the increase in antibiotic resistance. In fact, many other factors may affect H. pylori eradication. To analyze the clin. factors affecting the initial eradication therapy in Chinese patients with H. pylori infection. We conducted a retrospective study on 264 outpatients who were diagnosed with H. pylori-associated chronic gastritis and peptic ulcer disease between Jan. and Dec. 2015 at a large tertiary hospital in China. The patients were divided into three groups: ECA, RCA, and RCM (R: 20 mg rabeprazole, E: 40 mg esomeprazole, C: 0.5 g clarithromycin, A: 1.0 g amoxicillin and M: 0.4 g metronidazole). The patients were treated for 14 days and followed up for 1 yr. The 14C-urea breath test (14C-UBT) was performed 4 wk after the completion of the eradication therapy. The eradication rate was higher in 鈮?40-yr-old patients than in < 40-yr-old-patients (85.7% vs. 54.7%, p = 0.002). Multivariate analyses revealed only age 鈮?40 years to be significantly associated with a high H. pylori eradication rate [odds ratio (OR) 4.58, p = 0.003]. The H. pylori eradication rate in patients with duodenal ulcers was significantly higher than that in patients with gastric ulcers (79% vs. 60%, p = 0.012). In the FAS, esomeprazole was received by 57.1% of patients and was the most commonly used PPI. After 4-wk treatment, 71.1% of patients were considered responders to PPI. The response rate at the end of 2-wk PPI treatment reached 57.0% (95% CI, 52.5% to 61.7%). The median time to response was 13 days (95% CI, 12 to 15). Response rates at 2 and 4 wk of the per protocol set were similar to those of the FAS. Objective: To investigate the therapeutic effect of Bifidobacterium triple viable bacteria capsule combined with quadruple therapy on Helicobacter pylori (HP) pos. gastric ulcer (SU). Methods: A total of 50 HP-pos. SU patients admitted to our hospital from Jan. 2019 to Oct. 2020 were selected and divided into the control group and the observation group with 25 cases in each group by tossing a coin method. The control group was given quadruple therapy, and the observation group was given Bifidobacterium triple viable capsules orally on the basis of the control group. The curative effect, HP neg. rate and adverse reactions were compared between the two groups. Results: After treatment, the total effective rate of the observation group was significantly higher than that of the control group, and the difference was statistically significant (P<0.05). After treatment, 44 cases of HP in the observation group turned neg., and the neg. conversion rate was 89.80%. In the control group, HP turned neg. in 33 cases, and the neg. conversion rate was 68.75%. There was a statistically significant difference between the two groups (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: Bifidobacterium triple viable bacteria capsule combined with quadruple therapy is effective in the treatment of HP pos. SU, which can improve the HP neg. conversion rate and has good safety. Giardiasis represents a latent problem in public health due to the exceptionally pathogenic strategies of the parasite Giardia lamblia for evading the human immune system. Strains resistant to first-line drugs are also a challenge. Therefore, new antigiardial therapies are urgently needed. Here, we tested giardial arginine deiminase (GlADI) as a target against giardiasis. GlADI belongs to an essential pathway in Giardia for the synthesis of ATP, which is absent in humans. In silico docking with six thiol-reactive compounds was performed; four of which are approved drugs for humans. Recombinant GlADI was used in enzyme inhibition assays, and computational in silico predictions and spectroscopic studies were applied to follow the enzyme’s structural disturbance and identify possible effective drugs. Inhibition by modification of cysteines was corroborated using Ellman’s method. The efficacy of these drugs on parasite viability was assayed on Giardia trophozoites, along with the inhibition of the endogenous GlADI. The most potent drug against GlADI was assayed on Giardia encystment. The tested drugs inhibited the recombinant GlADI by modifying its cysteines and, potentially, by altering its 3D structure. Only rabeprazole and omeprazole decreased trophozoite survival by inhibiting endogenous GlADI, while rabeprazole also decreased the Giardia encystment rate. These findings demonstrate the potential of GlADI as a target against giardiasis. BACKGROUND: Gastritis is one of the common diseases, which is frequently caused by Helicobacter pylori. Triple therapy has resulted significant decrease in morbidity and complications. Newer proton pump inhibitor drug rabeprazole has been introduced in the market. The aim of this study is to compare its efficacy with omeprazole in triple therapy regimen. METHODS: A total of 100 patients who were positive for Helicobacter pylori and gave consent in participating in the study were included. Fifty patients were prescribed omeprazole-based triple therapy and other 50 were prescribed with rabeprazole-based triple therapy. After 2 weeks of triple therapy and 4 weeks of proton pump inhibitor treatment, Helicobacter pylori antigen was tested in faecal material. RESULTS: Out of 100 patients, there was significant correlation between epigastric pain, nausea and water brash with p value, 0.001. Similarly P-value was < 0.001 among hiatus hernia and reflux whereas p value was < 0.05 between bile reflux, hiatus hernia and reflux. In follow up study, after triple therapy, Helicobacter pylori antigen tests were negative in 94% of the study population, who were prescribed rabeprazole which was similar who were prescribed omeprazole (92%). CONCLUSIONS: Rabeprazole (20 mg) has proved similar Helicobacter pylori eradication rates compared with omeprazole (40 mg) when co-administered with of antibiotics (amoxicillin and clarithromycin) for two weeks.
